Why syphilis testing matters
Syphilis can be silent at first but cause serious health problems if untreated. Many tourists and expats in Bali choose routine screening after risky encounters.
VDRL vs TPHA
VDRL
Venereal Disease Research Laboratory
Screening test that detects non-specific antibodies. Can be positive in early or active infection.
TPHA
Treponema pallidum hemagglutination assay
Confirmatory test, more specific for syphilis antibodies.
When to test
Best performed 3–6 weeks after exposure. If you test earlier and results are negative, repeat later as advised.
Window period matters: Testing too early may give false negatives.

Syphilis is diagnosed with blood tests that look for antibodies. Two different kinds are usually combined:
- Non-treponemal test (VDRL or RPR) — shows whether infection is active. The result can be given as a titre (for example 1:16), which falls after successful treatment and is used to monitor recovery.
- Treponemal test (TPHA, TPPA or EIA) — confirms that antibodies to Treponema pallidum are present. This usually stays positive for life, even after cure.
| VDRL | TPHA | What it usually means |
|---|---|---|
| Negative | Negative | No infection — or tested too early. Repeat if exposure was under 6–12 weeks ago. |
| Positive | Positive | Current or recent syphilis, or an old infection that needs assessment. See a doctor for treatment. |
| Negative | Positive | Usually a past, treated infection — or very early infection. A doctor will review history. |
| Positive | Negative | Possible false positive (pregnancy, autoimmune conditions, some infections). Needs confirmation. |
Symptoms to Look Out For
- Primary stage (about 3 weeks after exposure): a single painless sore (chancre) on the genitals, anus or mouth. It heals on its own, so it is easily missed.
- Secondary stage (weeks to months later): a rash, often on the palms and soles, fever, swollen glands, sore throat or patchy hair loss.
- Latent stage: no symptoms at all, but the infection can still cause damage to the heart, brain and nerves years later.
When to Test
Most infections are detectable 3–6 weeks after exposure. If you test before 6 weeks and the result is negative, repeat the test at 12 weeks to be certain. Anyone with a painless genital or mouth sore should be tested straight away, whatever the timing.
Treatment
Syphilis is curable. Early syphilis is usually treated with a single injection of benzathine penicillin; alternatives exist for people with penicillin allergy. Sexual partners from the last three months (longer for later stages) should also be tested. A follow-up VDRL is usually done 6 and 12 months later to confirm the titre is falling.
Syphilis testing is included in our Premium STD package (IDR 4,600,000), or can be booked individually — see STD testing in Bali.
Frequently Asked Questions
How soon can syphilis be detected by a blood test?
Most infections show up 3–6 weeks after exposure. A negative test before 6 weeks should be repeated at 12 weeks.
What is the difference between VDRL and TPHA?
VDRL shows active infection and is used to monitor treatment; TPHA confirms syphilis antibodies and usually stays positive for life, even after cure.
Can syphilis be cured?
Yes. Early syphilis is usually cured with a single penicillin injection. Follow-up blood tests confirm the treatment worked.
Is syphilis testing confidential?
Yes. Anonymous testing is available at our clinic or at your villa, with no passport required.
